The phrase "dog tag" is also a colloquial or idiom indicating a form of identification tag similar to that worn by dogs that has (perhaps) name of animal, owner, phone number, address, vaccination record, etc.
The phrase was first popularized in the military and used to describe the metal tags worn around the neck by military personnel. It has since been co-opted by other organizations to describe most any kind of externally worn ID such as a name badge.
